USCIS / IMMIGRATION MEDICAL EXAM
Our USCIS Designated Civil Surgeons are experienced with the medical requirements of these USCIS IMMIGRATION MEDICAL EXAM / IMMIGRATION PHYSICALS. Individuals applying for citizenship, seeking Permanent Residency, or a Green Card are required to obtain an immigration physical prior to approval of their application. It is mandatory that immigration physicals be completed by a Designated Civil Surgeon like Dr. Vandana Bindal in order for the exam to be valid. We do all the necessary testing on site and have all of the required vaccines in our office. Internal Medicine Consultants are centrally located and serving near Beaverton, Hillsboro, Portland, Lake Oswego, Tigard, Aloha, West Linn & Tualatin OREGON.
Internal Medicine Consultants can perform the required immigration medical physicals in accordance with guidelines provided by the Department of Homeland Security, U.S. Citizen and Immigration Services, and Centers for Disease Control for immigrants or employers who hire employees originating from another country. The civil surgeon will record the exam results on Form I-693 for the applicant to return to the USCIS (INS). At Internal Medicine Consultants, USCIS Immigration Medical Exam / Immigration physicals are available Monday thru Friday. Internal Medicine Consultants performs every service you will need to pass the Immigration Medical Exam in first time.
Medical services needed to pass the USCIS Immigration medical / physicals exam:
- Physical Examination and completion of all the necessary paperwork
- Tuberculin (Skin Test) and interpretation
- RPR (Syphilis test)
- Evaluation of other Physical or Mental abnormality, disease or disability
- Vaccinations and status verification
- Administration of approved and needed vaccinations
Patients are advised to bring the following to their appointment:
- Valid Government issued photo ID such as a driver’s license or passport
- A copy of any prior immunization/vaccinations/shot records
- Prior chest x-rays and/or other diagnostic tests
- Any mental health records
- A completed version of the most recent Immigration Physical Form I-693
The USCIS Immigration Medical Exam Cost: Even with our convenient hours and service, our price for the Immigration Medical Exam cost is cheaper than most other doctors’ offices.
Prices are as follows:
- $250 for Immigration Physical Exam & Paperwork
- Additional fees for vaccines, labs and other services as required.
Required vaccinations are available for an additional fee or you can provide proof from another doctor.
Cash payment. The cost for the USCIS/green card medical exam is paid in cash, it is a self pay service.
After The Exam: Once the immigration medical / physical examination is completed, Internal Medicine Consultants will certify Form I-693 with the test results of the immigration exam and vaccination history. Please present this sealed envelope to Department of Homeland Security and U.S. Citizen and Immigration Services with your immigration application. You will also be provided with a copy of these forms. Please allow at least 1 week for the completion of these forms. We will contact you when they are ready to be picked up.

I used Dr. Bindal’s office to get my I-693 completed and had a great experience. The office managers and admin staff are very efficient and were able to accommodate me on short notice (they normally require appointments). They gave me very clear explanations / instructions and I was in and out of both appointments within an hour. Importantly, they made sure I got an opportunity to check the form for mistakes before they sealed it.
I called few places to get my I-693 filled, but most places could not give straight answers regarding vaccinations, wait time and total costs. So glad I found this place! They know what they’re doing, they do it with a positive attitude and they do it expeditiously (both times I went there were a handful of other clients). I went in on a Monday morning, was in and out in about 1 hour on that day. Came back on Wednesday morning for TB test reading, chest x-ray and tetanus shot, had my sealed report in hand in about 30 minutes. They will offer you alternatives to get things done elsewhere – for example shots you may be needing – for a lesser price, but they are fast and courteous.
